Originally the Xth Legion of the Great Crusade, the Iron Hands were forged by Ferrus Manus, a master smith and warlord whose hands were encased in living metal after slaying a legendary beast. During the Horus Heresy, Ferrus Manus was killed by his closest brother, Fulgrim of the Emperor’s Children, at the Drop Site Massacre on Isstvan V. This traumatic loss shaped the Chapter’s identity, fueling their belief that weakness—especially of the flesh—must be purged. They became bitter and reclusive, isolating themselves from other Chapters and embracing a doctrine of cold vengeance and mechanical purity.

The Iron Hands operate from the harsh world of ''Medusa'', where each of their ten Clan Companies maintains a mobile fortress-monastery. They have no single Chapter Master; instead, leadership is shared through the ''Iron Council'', often composed of Dreadnought-entombed veterans. Their warriors undergo extensive bionic enhancement, beginning with the ritual replacement of their left hand.
